摘要: Embodiments of the present invention provide improved methods and apparatus of treating the prostate. In many embodiments, at least a portion of the prostate is enucleated with a water jet, in order to decrease the invasiveness of the surgery. The access to the prostate can be provided with one or more of open surgical access, percutaneous access, or urethral access. The prostate can be enucleated such that an inner portion of the prostate comprising adenomatous tissue of the prostate is separated from a portion an outer the prostate comprising the capsule. In many embodiments, the patient is treated with energy of the water jet configured to separate the capsule from the inner tissue of the prostate and to inhibit cutting of blood vessels.

摘要: A method of neurohydrodissection for reducing a perception of pain in a patient is disclosed. The method comprises a step of physically inspecting the patient and locating a first compression site of a nerve of the patient where the nerve passes through a tissue and is inflamed. The method comprises a step of imaging the patient at the first compression site. The method comprises a step of inserting a syringe at the first compression site, wherein while the imaging the patient at the first compression site occurs, guiding the syringe to an intersection between the nerve and the tissue, injecting a solution with the syringe at the intersection and dissecting the nerve from the tissue.

摘要: A system for aspiration includes an aspiration catheter including an elongate shaft including an aspiration lumen, and an open distal end, an extension tube, the lumen of the extension tube configured to be hydraulically coupled to the aspiration lumen, a peristaltic pump configured for driving fluid through the extension tube, a compressible portion disposed between the distal end and the proximal end of the extension tube, the compressible portion configured to be coupled to the peristaltic pump, such that operation of the peristaltic pump drives fluid from the aspiration lumen from the distal end of the extension tube to the proximal end of the extension tube, and a controller configured to receive a first signal from the first sensor and configured to vary the operation of the peristaltic pump based at least in part on a first signal received from a first sensor coupled to the aspiration lumen or extension tube related to a change in the characteristic of flow.

摘要: A device for separating tissue in an eye includes a body configured to be positioned on the eye surface. The body receives air from one or more air supplies. The body includes one or more air supply channels. The separation device includes a plurality of needles extending from the body. Each needle includes a proximal opening, a distal opening, and a passageway extending between the proximal and distal openings. The proximal openings of the needles are coupled to the one or more air supply channels. The distal openings of the needles are spaced from the body to be positioned in eye tissue. The one or more air supply channels direct the air from the one or more air supplies into the proximal openings, through the passageways, and out the distal openings of the needles and into the eye tissue. The air applies a pressure to separate the eye tissue.

摘要: A laparoscopic instrument includes fluid carrying channels incorporated into lateral aspects of jaws of the instrument and extending to outlets adjacent distal tips of the jaws, fluid carrying channels incorporated into a shaft of the instrument, and flexible fluid carrying channels connecting the distal ends of the shaft channels with the proximal end of respective jaw channels. An actuator on the device handle controls either delivery of fluid jets emanating lateral to tissue grasped between the instrument jaws or suction from the distal tips of the jaws to remove fluid in the vicinity of the jaws. The fluid jets perform atraumatic tissue dissection lateral to the structures grasped by the jaws, while the grasping jaws provide counter traction while conducting soft tissue hydro-dissection, by stabilizing the tissue in a direction opposite to the force exerted by the hydro-dissection fluid jets.

摘要: Methods and systems for separating an object, such as a lead, from formed tissue are provided. Specifically, a tissue slitting device is configured to engage patient formed tissue at a slitting engagement point. While the object is subjected to a first traction force, the tissue slitting device is caused to move further into the engaged tissue and slit the tissue past the point of engagement. The slitting device causes the tissue to separate along an axial direction of the length of the formed tissue and releases at least some of the force containing the object. The methods and systems are well suited for use in cardiac pacing or defibrillator lead explant procedures.

摘要: A method of atraumatically hydrodissecting and maintaining endothelial function and structure of a vascular target includes forming an incision in tissue proximate one end to realize an insertion space, inserting a distal end of a cannula and/or endoscope into the insertion space and while visualizing the vascular target, ejecting a hydrodissecting fluid from the distal end of the cannula and/or endoscope to substantially separate or dissect the vascular target from the surrounding tissue, while advancing the distal end through the space as it is enlarged by the hydrodissecting fluid, to a distal target end of the vascular target. The hydrodissecting fluid is formulated to minimize or prevent formation of microthrombi in the hydrodissected vascular target. The hydrodissecting fluid is a water-based vascular graft treatment solution and can include any of a balance salt solution, a metallic salt solution, such as Plasma-Lyte® A, a vascular graft treatment solution, such as Duragraft® solution, L-Arginine, aspirin and low molecular weight heparin.

摘要: A treatment system for debriding a treatment area of a tissue site and applying negative pressure is disclosed. In some embodiments, the treatment system may include an ultrasonic bubble generator fluidly coupled to a negative-pressure source, fluid source, and a dressing. Fluid may be drawn from the fluid source to the ultrasonic bubble generator, whereby micro-bubbles and ultrasonic waves may be generated in the fluid before the fluid is instilled to the dressing.
