Abstract: Disclosed are various configurations of electrodes with accompanying extensions and wires configured to be attached at or near the left atrium of a heart to allow the device to be held snug against the endocardium and out of the blood flow for low energy defibrillation of the atria in response to atrial fibrillation or other atrial arrhythmias. The portion of the lead internal to the atrium (e.g., the left atrium) is restrained against the endocardium of the left atrium by way of a restraint mechanism. In one example, the electrode is configured to attach to the atrial septum, with wires containing memory-shaped metal to keep the wires against the heart wall. In yet another example, the electrode is configured to be part of a mitral valve device.

Abstract: A lead engagement device is configured to be positioned in a lead lumen. The lead engagement device includes a hypotube. The hypotube includes a wall having an inner surface defining an inner lumen and an outer surface. The wall defines a longitudinal axis of the hypotube. A plurality of lead engagement fingers extend outwardly and proximally from the outer surface at acute angles, and planes in which the acute angles are disposed extend diagonally relative to the longitudinal axis. The plurality of lead engagement fingers are configured to translate relative to the lead and permit relative motion between the lead engagement device and the lead when applying a first force to the lead engagement device. The plurality of lead engagement fingers are configured to engage the lead and inhibit relative motion between the lead engagement device and the lead when applying an opposite second force to the lead engagement device.

Abstract: A delivery device, a cardiac pacing device and a fixation structure are disclosed. The fixation structure includes a casing, a driving member and an elastic member. The casing has a first internal cavity and a slot, and the driving member is partially received in the first internal cavity in such a manner that one end of the driving member protrudes out of the first internal cavity from a proximal end thereof and is detachably connected to the driving sheath. The elastic member is accommodated in the first internal cavity in such a manner that its one end is coupled to the driving member and the other end extends outwardly from the driving member and is inserted in the slot. The driving member is configured for fitted connection with the casing while being able to move in an axial direction of the casing to drive the elastic member to move in the slot, thereby causing the elastic member to protrude out of or move back into the slot. As such, the cardiac pacing device can be fixed in a patient's body, allows retrieval and adjustment in pacing location, and features a simple structure, ease of operation, no limitation in tissue wall thickness and ease of fixation. Thus, the leadless pacing device can be fixed either in a ventricle or in an atrium to provide dual-chamber pacing and physiological pacing with atrioventricular synchronization.

Abstract: A method of selecting a combination of electrodes for use in neurostimulation includes testing combinations of electrodes in an electrode array to identify an atrial capture. The atrial capture is indicated by at least one effect on an atrium. The method includes excluding electrodes with the atrial capture to result in remaining electrodes of the electrode array, testing combinations of the remaining electrodes of the electrode array for effect on cardiac contractility and/or relaxation, and selecting a combination of electrodes from the remaining electrodes. The selected combination has both a desired effect on cardiac contractility and/or relaxation and does not have an undesired side effect.

Abstract: Methods and systems for separating an object, such as a lead, from formed tissue are provided. Specifically, a tissue slitting device is configured to engage patient formed tissue at a slitting engagement point. While the object is subjected to a first traction force, the tissue slitting device is caused to move further into the engaged tissue and slit the tissue past the point of engagement. The slitting device causes the tissue to separate along an axial direction of the length of the formed tissue and releases at least some of the force containing the object. The methods and systems are well suited for use in cardiac pacing or defibrillator lead explant procedures.

Abstract: This module comprises: a circuit for interfacing with the piezoelectric beam of an oscillating pendular unit, outputting a rectified signal comprising a sequence of pulses at a frequency equal to a multiple of the oscillation frequency of the pendular unit; a buffer capacitor charged by the successive pulses outputted by the interface circuit; and a converter regulator adapted to convert a capacitor discharge current into a stabilized power supply voltage, and controlled by a feedback control stage of the Maximum Power-Point Tracking (MPPT) type. A comparator detects the conduction of a blocking diode interposed between the interface circuit and the capacitor, in order to produce a signal representative of the current value of the duty cycle of the detected conduction and non-conduction periods. This signal is compared with a predetermined optimum duty cycle value in order to enable or disable the coupling of the capacitor to the converter regulator so as to control either the capacitor discharge towards an input of the converter regulator, or the continuation of its charging.
