A total hip joint prosthesis including an acetabular cup implant of a construction permitting an initial adjustable disposition for optimum articulation with a prosthetic femoral ball mounted on a femoral prosthesis within a usual cup implant, the implant is of a trapezoidal or truncated cone shape, and external fins and a porous surface are provided to facilitate an initial and long-term positional fixation in the user, and to compensate for physiological bone changes over an extended period of time. The porous surface is preferred but an option is to use a plain surface implant used as a "press-fitted" device.

An acetabular cup for use in a total hip prosthesis includes an implant/cement interface portion for location in a cement mantle. The cup has an enclosure element for allowing the cup to slide lengthwise to re-engage itself in the cement mantle to accommodate creep or movement thereof. Thus, should there be a tendency for the cup to become loose, the construction according to the invention is intended to relock the cup within the cement mantle. Thus, if the bone remodels, then the cement mantle will creep to re-adapt itself to the bone and the implant will re-engage in the cement mantle accommodating to the movement of the cement. If the bone cement is too highly stressed in places, it will creep in order to relieve this stress and the implant will re-settle.

The socket body has an exterior surface which tapers in one direction while anchoring beads on the exterior surface taper in an opposite outward direction. The beads serve to anchor the body within a pelvis. In addition, cross ribs are formed on the anchoring beads with at least some of the ribs being elastic. Upon insertion of the socket, the ribs bend in a direction opposite to the direction of insertion. The two oppositely directed anchoring tapers of the socket body and the beads provide a firm fit of the socket body in a pelvis with a cement-free anchoring. Also, the bent ribs tend to resist withdrawal of the socket from the pelvis.
