摘要: The method allows controlling the quality of an initial RR series made up of a plurality of (RRi) samples which are respectively a function of time intervals (δti) which separate two successive heartbeats. During this method, one resamples the RR series so as to obtain a resampled RR series, and one automatically controls the quality of the RR series by automatically calculating at least the mathematical norm value (NORME), in a sliding window, of the resampled RR series, said mathematical norm value being given by the following formula: NORME = ∑ i = 1 N ( RR i - 1 N ∑ i = 1 N ( RR i ) ) 2 where N is the number of RRi samples in said window.

摘要: New and alternative approaches to the monitoring of cardiac signal quality for external and/or implantable cardiac devices. In one example, signal quality is monitored continuously or in response to a triggering event or condition and, upon identification of a reduction in signal quality, a device may reconfigure its sensing state. In another example, one or more trends of signal quality are monitored by a device, either continuously or in response to a triggering event or condition, and sensing reconfiguration may be performed in response to identified trends and events. In yet another example, a device may use a looping data capture mode to track sensing data in multiple vectors while primarily relying on less than all sensing vectors to make decisions and, in response to a triggering event or condition, the looped data can be analyzed automatically, without waiting for additional data capture to reconfigure sensing upon identification of the triggering event or condition. In another example a device calculates a composite cardiac cycle by overlaying signal morphology for a number of cardiac cycles and analyzes the composite cardiac cycle to calculate signal quality metrics.

摘要: In some examples, a system can be used for delivering cardiac resynchronization therapy (CRT). The system may include a pacing device configured to be implanted within a patient. The pacing device can include a plurality of electrodes, signal generation circuitry configured to deliver ventricular pacing via the plurality of electrodes, and a sensor configured to produce a signal that indicates mechanical activity of the heart. Processing circuitry can be configured to identify one or more features of a cardiac contraction within the signal, and determine whether the contraction was a fusion beat based on the one or more features.

摘要: Techniques are described for discriminating SVT and, in particular, rapidly conducting AF. The techniques include detecting an onset of a fast rate of ventricular events sensed from a cardiac electrical signal and detecting a pause in the fast rate of ventricular sensed events. A threshold number of ventricular event intervals required to detect a ventricular tachyarrhythmia is detected with each of the threshold number of ventricular event intervals being less than a tachyarrhythmia detection interval. Detection of the ventricular tachyarrhythmia and an electrical stimulation therapy for treating the ventricular tachyarrhythmia are withheld in response to at least the pause being detected.

摘要: The present invention relates to a determination system, a corresponding determination method and a computer program for determining a sleep stage of a subject. The determination system comprises a cardiorespiratory signal providing unit (10), an actigraphy data providing unit (20), a first sleep stage determination unit (30) for determining the sleep stage based on a cardiorespiratory signal, and a second sleep stage determination unit (40) for determining the sleep stage based on actigraphy data. The first sleep stage determination unit (30) determines a sleep onset latency (SOL) of the subject. The sleep stage of the subject is determined using the first sleep stage determination unit (30) up to the SOL and the second sleep stage determination unit (40) after the SOL. The determination system and corresponding method allow for a more reliable determination of a sleep stage of subject, in particular improve the determination of the SOL of the subject.

摘要: A system and method for facilitating a cardiac rhythm disorder diagnosis based on subcutaneous cardiac monitoring data with the aid of a digital computer are provided. Cutaneous action potentials of a patient are recorded as electrocardiogram (EGC) data over a set time period using a subcutaneous insertable cardiac monitor. A set of R-wave peaks is identified within the ECG data and an R-R interval plot is constructed. A difference between recording times of successive pairs of the R-wave peaks in the set is determined. A heart rate associated with each difference is also determined. The pairs of the R-wave peaks and associated heart rate are plotted as the R-R interval plot. A diagnosis of cardiac disorder is facilitated based on patterns of the plotted pairs of the R-wave peaks and the associated heart rates in the R-R interval plot.

摘要: In accordance with an aspect of the present invention, a system and method allows for the assessment of health and mortality based on dynamic nonlinear calculations of self-similar fluctuation patterns in a time series of QT intervals and of other physiological signals, such as RR intervals, temperature, blood pressure, respiration, saturation of peripheral oxygen, intracardiac pressures, and electroencephalogram. In order to nonlinearly determine health and mortality, time series of QT intervals and of other physiological signals (e.g., RR intervals) are simultaneously obtained, and entropy values are calculated for each signal over the same temporal interval. “EntropyX” is calculated from relative changes between moments and entropy of QT intervals and those of other physiological signals over seconds to days. The absolute and relative entropy values at a specific time point and/or subsequent changes in entropy over future time points can be used to determine a treatment plan for the subject.
