Abstract: Embodiments herein relate to implantable medical devices including a power subunit with a first biocompatible electrically conductive shell configured for direct contact with an in vivo environment. In some embodiments a lithium anode can be disposed within the first biocompatible electrically conductive shell in direct electrical communication with a feedthrough pin, wherein the feedthrough pin is electrically isolated from the first biocompatible electrically conductive shell. A cathode can also be disposed within the first biocompatible electrically conductive shell and can be in direct electrical communication with the first biocompatible electrically conductive shell. The first biocompatible electrically conductive shell has a positive electrical potential. The implantable medical device further includes an electronics control subunit with a control circuit disposed within a second biocompatible electrically conductive shell. Abstract: Systems and methods are disclosed to an ambulatory medical device comprising a cardiac signal sensing circuit configured to sense a cardiac signal representative of cardiac activity of a patient when connected to electrodes, and a control circuit. The control circuit is configured to monitor cardiac depolarizations in the sensed cardiac signal, detect a bimodal distribution of heart rate of the patient, identify cardiac depolarization intervals shorter than a predetermined interval threshold, identify premature atrial contractions (PACs) in the sensed cardiac signal that are conducted normally and conducted aberrantly, and count a number of conducted PACs and produce an alert related to PC burden of the patient based on the number.

Abstract: Systems, methods, and devices involve approaches for limiting the number of detected cardiac events for which a physician is alerted. Approaches involve detecting cardiac events based on electrocardiogram (ECG) data and associating the cardiac events with a criticality. ECG data associated with a subset of the cardiac events is transmitted to a remote computing system based, at least in part, on the criticality. An electronic notification is transmitted to a patient care group regarding at least one of the cardiac events in the subset. Approaches further include determining whether an acknowledgement of the electronic notification has been received by the server from the patient care group within a set period of time.

Abstract: Systems and methods for monitoring and managing aortic stenosis are disclosed. An exemplary medical-device system comprises a data receiver to receive heart sound information sensed by ambulatory heart sound sensors, and a stenosis detector to detect a change in aortic valve surface area from a baseline stenosis-free state using heart sound components such as S1 and S2 sounds. Based on the change in aortic valve surface area, the stenosis detector can generate an aortic stenosis indicator indicating a presence and severity of aortic stenosis. Such indicator can be provided to a user, trigger symptom monitoring and evaluation of functional deterioration in the patient, facilitate assessment of patient candidacy for aortic valve replacement, or triage heart failure management strategies.

Abstract: Systems and methods for monitoring and treating patients with heart failure are discussed. The system may receive patient atrioventricular (AV) conduction characteristic under different heart rates or patient conditions. Stimulation parameters including stimulation timing parameters may be stored in a memory. The system may include a stimulation control circuit configured to determine a parameter update schedule indicating a timing at which to update stimulation parameter using patient AV conduction characteristic, and dynamically update at least a portion of the stored set of stimulation parameters at the determined parameter update schedule. For a specified heart rate or heart rate range, a stimulation parameter may be selected from the set of the stimulation parameters for use during cardiac stimulation.

Abstract: Hydrogen getters are provided in active implantable medical device by applying a layer of selected metal with a chosen thickness over an un-oxidized portion of a base metal. The base metal may be titanium, and the selected metal may be palladium or platinum, with a thickness of less than 250 nanometers, where the selected metal acts as a passivation layer relative to oxidation but allows hydrogen capture by the base metal. The getter may be a separate component or may be formed as part of an existing component such as a feedthrough ferrule.

Abstract: Improved devices, circuits and methods of operation in implantable stimulus systems. An implantable defibrillator may comprise a charging circuit using a transformer to store and build up energy on an HV capacitor or capacitor stack, with the HV capacitor in turn coupled to an H-bridge output circuit having low and high sides for issuing therapy. A current monitoring circuitry is provided on the low side of the H-Bridge and used to form a feedback loop to control current into a transformer that converts battery voltage to a signal that charges the HV capacitor to control current through the H-bridge.
