    摘要: The invention provides for a medical instrument (300, 500, 600) comprising a high intensity focused ultrasound system (302) comprising an ultrasonic transducer (306) with multiple transducer elements (400, 402, 404, 406, 408). The medical instrument further comprises a memory (334) containing machine executable instructions (350, 352, 354, 520, 522, 524) which cause a processor to receive (100, 200) a treatment plan (340) specifying a protected zone (322) within a subject (301) and to calculate (102, 208) a set of transducer control parameters (342) using the treatment plan. The set of transducer control parameters specify the switching of electrical power to the multiple transducer elements. An ultrasonic intensity estimate (900) in the protected zone is below a predetermined threshold. The ultrasonic intensity estimate is calculated using an incoherent sum of the ultrasonic pressure generated by each of the multiple transducer elements.

    摘要: An ultrasonic surgical instrument includes an elongate substantially rigid probe and an elongate tubular sheath member. The probe is operatively connected at a proximal end to a source of ultrasonic mechanical vibrations and has a distal tip configured for transmitting ultrasonic vibration energy into organic tissues. The probe longitudinally traverses the sheath member. The sheath member has a distal edge with a first portion on one side of the probe and a second portion on an opposite side of the probe. The first portion of the distal edge of the sheath member is disposed substantially closer than the second portion of the sheath's distal edge to the distal tip of the probe. The second portion of the distal edge is so spaced from the distal tip as to permit effective visualization of the distal tip during use of the instrument.

The disclosure describes the use of ultrasound devices consisting of an ultrasound generator, ultrasound transducer and ultrasound horn at least partially enclosed by a shield to enhance neurogenesis on neurons of the central nervous system and other nerve tissue. The shield allows the application of therapeutic levels of ultrasound to nerve tissue to enhance the regeneration of the nerve tissue and mitigate damage to the nerve tissue. The shield serves to protect patient from unwanted contact with portions of the ultrasound horn not being utilized for therapeutic effect. The ultrasound application may be included in devices that have other uses such as a cutting edge for removing tissue.
