The paddle lead comprises two flexible concave paddle bodies that are joined together at their opposing convex surfaces. The first paddle body contains a series of electrodes that are embedded on the concave surface that expand to fit the contours of the dura mater. The second paddle body consists of a concave surface that is pressed against the bone of the spinal column to act as a fixation mechanism to keep the paddle assembly in place.

    摘要: A medical device for placing a medical lead in the human body using minimally invasive techniques is described. One lead includes a lead body connected to a lead head having an aperture for providing fiber optic access to the interior of a helical electrode. The fiber optic shaft may be disposed within or along-side a drive shaft releasably coupled to the head to rotate the head. The drive shaft and lead body may be delivered using a delivery catheter. The delivery catheter can be advanced though a small incision to the target tissue site, and the site remotely visualized through the fiber optic scope extending through the lead head aperture. Some catheters include a distal mapping electrode readable from the catheter proximal portion or handle. The lead head can be rotated, rotating the helical electrode into the tissue, and the catheter, drive shaft, and fiber optic probe removed. In one use, epicardial pacing leads are placed on the posterior surface of the heart, aided by visualization and mapping to obtain optimal electrode placement and patient outcome.

The fixation mechanism may include one or more expandable wire-like elements, which may be configured in a substantial helical shape. The wire-like elements may be formed from an elastic or super-elastic material, and expand radially outward when a restraint mechanism is removed following implantation of the lead.

    摘要: An implantable medical electrical lead particularly for stimulation of the sacral nerves comprises a lead body extending between a distal end and a proximal end, and the distal end having at least one electrode of an electrode array extending longitudinally from the distal end toward the proximal end. The lead body at its proximal end may be coupled to a pulse generator, additional intermediate wiring, or other stimulation device. A fixation mechanism is formed on or integrally with the lead body proximal to the electrode array that is adapted to be implanted in and engage subcutaneous tissue, particularly muscle tissue, to inhibit axial movement of the lead body and dislodgement of the stimulation electrodes.

    摘要: In general, the invention provides a transurethral ablation device comprising an electrode for delivery of neurostimulation therapy to a patient to prevent or alleviate pain experienced during the course of a transurethral ablation procedure. The device may include a conductive ablation needle that delivers electrical current for ablation of prostate tissue, as well as neurostimulation current to prevent or alleviate pain encountered in response to the ablation current. In this sense, the ablation needle is integrated with a neurostimulation electrode. Alternatively, the device may incorporate a dedicated neurostimulation electrode independently of the ablation needle, either on the needle or on a catheter body that carries the needle.

    摘要: In general, the invention is directed to treatment of urinary incontinence by the implantation of one or more bulking prostheses proximate to a urethral sphincter. These bulking prostheses, which may include biocompatible hydrogel, are implanted into the tissue outside the urethra, proximate to a urethral sphincter. When implanted, the bulking prostheses are in a miniature state. Upon introduction into the body, the devices enter an enlarged state. In their enlarged state, the bulking prostheses supply extra bulk to the tissues proximate to the external urethral sphincter. With the extra bulk, the patient can exercise voluntary control over the external urethral sphincter to close or maintain closure of the urethra and maintain urinary continence.

    摘要: Lead devices and methods for neurostimulation are described. Some of the neurostimulation leads provided have multiple, distal region electrode contacts arranged in an array capable of providing steerable spinal cord stimulation. In one use, a collapsed introducer sheath is disposed over a dilator and advanced to a spinal cord target site through a 14-gauge needle into the epidural space. The dilator is removed and the introducer is expanded to its full width. A neurological lead according to the present invention advanced through the introducer to near the target site and the introducer removed. The lead is wider than it is high and has a row of lower electrodes flanked by right and left side electrodes in groups of three. The lower electrodes serve as cathodes and the right and left electrodes are return anodes.

For example, a tissue ablation device having an actuator, such as a trigger mechanism, coupled to a power source and an electrode is discussed. A single step of engaging the actuator causes the electrode to be introduced into the target tissue and causes energy to be delivered from the power supply to the tissue via the electrode. Devices that include an electrode actuator for causing the electrode to be introduced into the target tissue and a fluid actuator for causing the fluid to flow to the target tissue are also discussed. Methods of causing the electrode to be introduced into the target tissue and fluid to flow to the target tissue with a single step and more than one step are also discussed.

    摘要: An electrical stimulation lead incorporating an electrode assembly which dispenses a drug adjacent the stimulation site. The lead is provided with a fixation helix located at the distal end of the lead. An electrode may be extended distally within the helix. When extended, the electrode serves as a guard against perforation or other damage to tissue by the helix. In its retracted position, the electrode is located roughly adjacent the distal end of the lead body, and lies against the tissue to be stimulated. Advancing and retraction of the electrode is accomplished by means of a stylet which is removably engageable with the proximal end of the electrode. In one embodiment, the stylet functions to both advance and retract the electrode. In an alternative embodiment, the stylet is used only to advance the electrode, and retraction of the electrode is accomplished by a resilient conductor.
