Abstract: Suture passers and methods of use. Described herein are suture passers preloaded with suture, including cartridges that couple to a suture passer to form a loaded suture passer that are configured to prevent a length of suture (e.g., already-passed suture) from re-entering the channel of the suture passer jaw and getting recaptured and/or entangled by the tissue penetrator. In particular, described herein are preloaded and automatically re-loading suture passers that are adapted to include a gate, guide or shield on one of the jaws of the suture passer, and methods of operating such suture passers for surgical use including repairing tissue.

Abstract: A surgical buttress loading system includes an anvil having a spring assembly biased against a tissue facing surface of an anvil body and a loading tool including a cavity configured to receive the anvil therein, a buttress cartridge including a surgical buttress releasably disposed thereon, an actuator, and a handle operably coupled to the buttress cartridge. When the anvil is positioned within the cavity of the loading tool and the handle is moved to from an unactuated position to an actuated position, the handle moves the actuator to from a retracted position to an extended position and into engagement with the spring assembly of the anvil to lift the spring assembly off of the tissue facing surface and then slides the buttress cartridge towards the spring assembly to move a proximal end portion of the surgical buttress between the spring assembly and the tissue facing surface.

Abstract: A fastener cartridge assembly for use with a surgical end effector. The fastener cartridge assembly can comprise a cartridge body having fastener cavities, fasteners removably positioned in the fastener cavities, a layer of material releasably secured relative to the cartridge body, a connector configured to secure the layer of material to the cartridge body at a location distal to at least one fastener cavity, and an actuator. The layer of material can be a tissue thickness compensator. When actuated, the actuator can overcome the connector, and can overcome the connector prior to the removal of the fasteners from the fastener cavities. The actuator can be actuated at a location proximal to at least one fastener cavity, and can extend distally past at least one fastener cavity toward the connector.

Abstract: A two-part tissue thickness compensator assembly can include a first tissue thickness compensator configured to be positioned relative to an anvil of a surgical stapler, a second tissue thickness compensator configured to be positioned relative to a staple cartridge of the surgical stapler, and a hinge connecting the first tissue thickness compensator to the second tissue thickness compensator. The first and/or second tissue thickness compensators may include additional engagement features, such as a raised ridge that engages a slot in the anvil and/or the staple cartridge. In certain embodiments, the first and/or second tissue thickness compensators may include an encasement that contains a suitable biologic agent. An end effector assembly may be provided for attachment to a surgical instrument that includes, for example, a staple cartridge, an anvil, a first tissue thickness compensator positioned on the anvil, and a second tissue thickness compensator positioned on the staple cartridge.

Abstract: Devices, methods and systems are provided for occluding an opening within the tissue of a body, such as a left atrial appendage. In one embodiment, a delivery system for use in occluding an opening in the tissue of a body includes an actuation assembly operatively coupled to a medical device including an anchor portion and an occluder portion. The actuation assembly is configured to move the anchor portion between a deployed state and retracted state while the occluder portion maintains a deployed state. With this arrangement, the deployed occluder portion can be visualized via imaging at a preferred position prior to deploying the anchor portion of the medical device.

Abstract: A device for delivering an occlusive element includes an elongate pusher member having a lumen. A locking member is disposed within the lumen of the elongate pusher member. A moveable elongate releasing member is disposed within the lumen of the elongate pusher member. A filament is secured to the distal end of the elongate releasing member. The occlusive member is locked to the elongate releasing member when the filament passes through a securing member on the occlusive member and is pinched or wedged between the looking member and the elongate releasing member. The occlusive element is in an unlocked state when the elongate releasing member is retracted proximally relative to the elongate pusher member. The filament, along with the elongate releasing member, are retracted proximally until the filament is detached or uncoupled from the securing member of the occlusive member.

Abstract: A tissue thickness compensator comprising at least one woven lattice can be positioned in the end effector of a surgical instrument. A fastener cartridge that is positioned in the end effector can comprise at least one cavity configured to receive a fastener. The fastener can be moveable between an initial position, wherein the fastener is at least partially position in a cavity, and a fired position, wherein the fastener is configured to compress a woven lattice of the tissue thickness compensator. The woven lattice can comprise a resilient material such that compression of the woven lattice generates a restoring force. The woven lattice can also comprise an axis that can laterally traverse the fastener cartridge, diagonally traverse the fastener cartridge, or intersect a deck surface of the fastener cartridge. The woven lattice can comprise a hydrophilic substance, which can expand when the woven lattice is severed by a cutting element.
