An implantable stimulation lead is disclosed for placement in the coronary sinus region and its associated coronary vessels overlying the left side of a patient's heart. The lead comprises at least one proximal connector; at least one tissue stimulation electrode; at least one conductor coupled between the at least one proximal connector and the at least one stimulation electrode; and a lead body including a housing of insulating material enclosing the at least one conductor, the lead body having a relatively flexible distal portion of, for example, silicone rubber, having a length corresponding to the coronary sinus region of the heart, and a stiffer proximal portion of, for example, polyurethane. A robust transition joint comprising telescoped sections of the distal and proximal portions of the lead body couples the two portions of the lead body.Also provided is a versatile lead delivery system including a stylet stop disposed within the distal portion of the lead body. The stylet stop defines an aperture dimensioned to pass a guide wire but not the enlarged distal tip of a stylet. The lead includes a tip electrode having a longitudinally extending bore dimensioned to permit passage of the guide wire through the tip electrode.

An implantable cardiac stimulation system senses electrical activity of all four chambers of the heart and delivers pacing and defibrillation pulses to all four chambers of the heart. The system includes a lead system consisting of first and second leads. The second lead is implantable in the coronary sinus of the heart and includes a left ventricular pacing electrode, a left ventricular defibrillation electrode, a left atrial pacing electrode, and a left atrial defibrillation electrode. A right atrial pacing electrode is carried by one of the first and second leads. The system further includes a cardiac stimulation device including a pulse generator that delivers defibrillation pulses between any combination of the defibrillation electrodes and pacing pulses to any one of the pacing electrodes and a sensing circuit that senses cardiac electrical activity with any one of the pacing electrodes.

A cardiac stimulation device, a His Bundle lead, and an associated method for detecting a conduction signal from the His Bundle. Upon this detection, the stimulation device delivers ventricular stimulation to the right and/or left ventricles at an interval following the atrial depolarization. A sensing window is initiated following the sensing of the atrial depolarization. During the sensing windows, the His Bundle signal received through the His Bundle lead is sampled and a moving average is calculated to determine a signal peak. Detection of the His Bundle signal peak triggers ventricular stimulation, which may be delivered to the right ventricle, the left ventricle, or both ventricles.

A suture sleeve for anchoring the lead body of an implantable medical device is fabricated of a biocompatible, polymeric material to which has been added color pigment in clear contrast with the bodily tissue adjacent the sleeve. The sleeve is therefore readily visible to the naked eye to aid the physician in locating the sleeve during lead implant or lead revision. Furthermore, the tubular body may be radiopaque and, therefore, easily seen on a fluoroscope through the addition of a radiologically dense material, such as tantalum powder, to the plastic from which the sleeve is molded.

A programmable output connector of an implantable cardioverter-defibrillator (ICD), or similar implantable medical device, allows each of a multiplicity of output terminals to be selectively connected to either a positive or a negative output bus of the ICD. The positive and negative output buses of the ICD, in turn, are switched to an output capacitor, or equivalent output circuit, of the ICD. An electrical charge stored on the output capacitor, or otherwise generated by the output circuit, is presented to the multiplicity of output terminals in accordance with a programmed polarity. The programmed polarity causes a selected one or group of the multiplicity of output terminals to be connected to the positive output bus, and a selected other or group of the multiplicity of output terminals to be connected to the negative output bus. Respective electrodes designed for contacting cardiac tissue may then be electrically connected to each one of the multiplicity of output terminals. An electrical charge available on the positive and negative output buses may then be transferred to the body tissue as an electrical shock or stimulus through the electrodes in accordance with the programmed polarity.

When the tissue fixation helical anchor is progressively embedded in the cardiac tissue, the cardiac tissue progressively displaces the structure distal end proximally.
