Abstract: The present disclosure relates generally to pacing of cardiac tissue, and more particularly to adjusting delivery of His bundle or bundle branch pacing in a cardiac pacing system to achieve synchronized ventricular activation. A left bundle branch (LBB) cathode electrode may be implanted a left side of the septum of the patient's heart proximate to the LBB, and a right bundle branch (RBB) cathode electrode may be implanted on a right side of the septum of the patient's heart proximate to the RBB. One or both cathode electrodes may be used to deliver synchronized left and right bundle-branch pacing based on one or both of an atrial event and a ventricular event. A device for bundle branch pacing may be implanted based on determining whether an LBB block pattern or an RBB block pattern is present in monitored electrical activity.

Abstract: Various techniques include advancing an implantable medical device comprising a tissue-piercing electrode toward the triangle of Koch region of the right atrium or through the coronary sinus to deliver cardiac therapy to or sense electrical activity of the left ventricle in the basal region, septal region, or basal-septal region of the left ventricular myocardium of a patient's heart. The tissue-piercing electrode may include an imagable material. The implantable medical device may include an imagable member made of an imagable material coupled to a housing or a lead of the implantable medical device. Image information representing the patient's heart and the imagable material may be acquired using two-dimensional imaging. Orientation information representing the implantable medical device may be generated based on the acquired image information.

Abstract: Implantable medical systems enter an exposure mode of operation, either manually via a down linked programming instruction or by automatic detection by the implantable system of exposure to a magnetic disturbance. A controller then determines the appropriate exposure mode by considering various pieces of information including the device type including whether the device has defibrillation capability, pre-exposure mode of therapy including which chambers have been paced, and pre-exposure cardiac activity that is either intrinsic or paced rates. Additional considerations may include determining whether a sensed rate during the exposure mode is physiologic or artificially produced by the magnetic disturbance. When the sensed rate is physiologic, then the controller uses the sensed rate to trigger pacing and otherwise uses asynchronous pacing at a fixed rate.

Abstract: Transcardiac diaphragmatic stimulation includes detecting a cardiac event based on signals sensed through a cardiac event sensor located in or on a region of a heart in proximity to a diaphragm, and delivering an ADS therapy through an ADS therapy mechanism that is located in proximity with the region of the heart, to induce a contraction of the diaphragm without inducing a contraction of the heart. The cardiac event sensor may be located a) on an interior surface of a cardiac wall that abuts the diaphragm, or 2) on an exterior surface of the heart, between a cardiac wall and the diaphragm. The ADS therapy mechanism may be located: a) on an interior surface of a cardiac wall that abuts the diaphragm, 2) on a superior surface of the diaphragm that abuts a cardiac wall, or 3) on an inferior surface of the diaphragm at a region of the diaphragm that abuts the heart.

Abstract: Systems and methods for controlling blood pressure by controlling atrial pressure and atrial stretch are disclosed. In some embodiments, a stimulation circuit may be configured to deliver a stimulation pulse to at least one cardiac chamber of a heart of a patient, and at least one controller may be configured to execute delivery of one or more stimulation patterns of stimulation pulses to the at least one cardiac chamber, wherein at least one of the stimulation pulses stimulates the heart such that an atrial pressure resulting from atrial contraction of an atrium overlaps in time a passive pressure build-up of the atrium, such that an atrial pressure of the atrium resulting from the stimulation is a combination of the atrial pressure resulting from atrial contraction and the passive pressure build-up and is higher than an atrial pressure of the atrium would be without the stimulation, and such that the blood pressure of the patient is reduced.

Abstract: In some examples, controlling delivery of cardiac resynchronization therapy (CRT) includes storing, in a memory of an implantable medical device system and in association with each of a plurality of heart rates, at least one respective value for an interval between an atrial event and a ventricular event. Processing circuitry of the implantable medical device system may determine a heart rate of a patient and select one of the stored values for the interval between the atrial event and the ventricular event associated with the determined heart rate. The processing circuitry may further determine whether to control therapy delivery circuitry of the implantable medical device system to deliver fusion pacing or biventricular pacing, based on the selected one of the stored values for the interval between the atrial event and the ventricular event.
