    摘要: A method of achieving gas induced production of liquid from wells wherein the wells have a production tubing string extending to a production formation wherein such wells may have insufficient gas production for achieving continuous production of liquids from such wells. The method includes establishing a liquid accumulation chamber being located downstream of a gas accumulation chamber and being in communication with the tubing string, while the gas accumulation chamber is in communication with the production formation. Liquid from the production formation is produced by maintaining a condition of predetermined low flow of gas and liquid produced from the production formation. The method includes accumulating a predetermined quantity of liquid within the liquid accumulation chamber during the predetermined low flow condition, while simultaneously accumulating a quantity of gas within the gas accumulation chamber. Upon accumulation of the predetermined volume of liquid within the liquid accumulation chamber, a quantity of gas from the gas accumulation chamber is suddenly released into the liquid accumulation chamber, transporting a portion of the accumulated liquid from the liquid accumulation chamber upwardly through the tubing string for production thereof. Suitable apparatus for achieving the method of the present invention may include a flow control mechanism connected to the tubing string and cooperating with the tubing string to define the liquid accumulation chamber and the gas accumulation chamber. Movable orifice restrictor means may be connected to the flow control means and may be automatically movable between a first position establishing the condition of low flow and a second position establishing a condition of increased flow. The movable orifice restrictor means may be movable to the first position thereof by the mass flow rate of the flowing production fluid medium and may be movable to the second position thereof by urging means in response to accumulation of the predetermined quantity of liquid within the liquid accumulation chamber.

    摘要: A method of endoscopically removing tissue including providing a surgical cutting instrument including a first elongated member having a cutting tip, a second tubular member forming a cutting window, and a handpiece. The first elongated member is co-axially disposed within the second tubular member such that the cutting tip is exposed at the cutting window and combines to define a cutting implement. The first and second members are coupled to the handpiece. The method includes grasping the handpiece, deploying the instrument such that the cutting implement is adjacent a target site with the cutting window positioned at a first spatial orientation relative to the handpiece, rotating the cutting window relative to the handpiece such that the cutting window is positioned at a second spatial orientation, and moving the first elongated member relative to the second tubular member such that the cutting tip removes tissue from the target site.

    摘要: A surgical cutting instrument including a first elongated member, a second tubular member, a handpiece, a first hub, a second hub, and an actuator assembly. The first tubular member has cutting tip and is co-axially disposed within the second tubular member such that the cutting tip is exposed at a cutting window. The first hub is mounted to the first tubular member and is moveably coupled to the handpiece. The second hub is mounted to the second tubular member and defines a hub axis. The actuator assembly rotatably couples the second hub to the handpiece and includes an actuator. Movement of the actuator is translated to rotational movement of the second hub so as to effectuate spatial rotation of the cutting window. In this regard, the actuator movement is not co-axial with the hub axis.

    摘要: A method of performing sinus surgery utilizes a sinus debrider instrument having an outer tubular member, an inner member rotatably disposed within the outer member and carrying a tissue cutting surface at a distal end thereof, a suction passage through the instrument for removing cut tissue from a sinus and an annular space between the inner and outer members forming a fluid passage for supplying fluid to the tissue cutting surface. The method includes the steps of positioning the distal end of the instrument at an operative site within the sinus, cutting tissue at the operative site within the sinus by rotating the tissue cutting surface, removing the cut tissue from the sinus through the suction passage and supplying fluid to the tissue cutting surface through the fluid passage to facilitate the removal of cut tissue without introducing fluid to the operative site.

    摘要: A method of making a cut in the nasal bone of a patient includes the steps of reciprocating a distal end of an osteotome via a powered surgical handpiece and moving the distal end of the osteotome, while it is being reciprocated, forwardly along the nasal bone of a patient in a predetermined path with a cutting edge of the osteotome in contact with the nasal bone to make a cut of desired length in the nasal bone along the predetermined path. The distal end of the osteotome may be moved so as to produce a straight cut and/or a curved cut in the nasal bone. The distal end of the osteotome may be reciprocated by converting rotary motion of the handpiece into reciprocating motion of the osteotome.

    摘要: A surgical handpiece adapter for converting rotary motion of a powered surgical handpiece into reciprocating motion to drive a cutting member includes a rear drive shaft for being removably coupled to a rotatable drive shaft of the handpiece, a front drive shaft for being removably coupled to the cutting member and a motion converting mechanism causing reciprocation of the front drive shaft and, therefore, the cutting member coupled thereto, in response to rotation of the rear drive shaft by the drive shaft of the handpiece. A handpiece adapter assembly is formed by an adapter and a cutting member coupled thereto, and a powered surgical handpiece assembly is formed by an adapter, a cutting member coupled to the adapter and a powered surgical handpiece coupled to the adapter. A cutting member includes a surgical suction rasp for being reciprocatively driven to cut anatomical tissue and having a tissue cutting surface and a suction passage with an inlet opening along the tissue cutting surface for removing anatomical debris from an operative site at which the rasp is used. A cutting member includes an osteotome for being reciprocatively power driven to cut anatomical tissue and having a cutting edge and a blunt tip extending distally of the cutting edge. A method of facial surgery includes the steps of reciprocating a distal end of a rasp to reshape the nasal bone of a patient and removing anatomical debris through a suction passage of the rasp while the nasal bone is being reshaped. Another method of facial surgery includes the steps of reciprocating a distal end of an osteotome via a powered surgical handpiece and moving the distal end of the osteotome, while it is being reciprocated, forwardly along the nasal bone of a patient in a predetermined path with a cutting edge of the osteotome in contact with the nasal bone to make a cut of desired length in the nasal bone along the predetermined path.

    摘要: The adapter member for a microscope cooperates with a universal microscope drape having a universal positioning member. The adapter member is detachably secured to the lens housing of a microscope in tandem with a lens unit located in the lens housing. The adapter member is free from connection with the lens unit and can be installed without disassembling the lens housing. In one embodiment of the invention, the adapter member is threaded to the lens housing and in another embodiment of the invention the adapter member is secured to the lens housing with set screws. The adapter member has a universal engagement portion that is engagable with the universal positioning member of the universal microscope drape. A different adapter member is provided for each differently manufactured microscope but each different adapter member has the same universal engagement portion. Thus the same microscope drape can be used to cover differently manufactured microscopes of the same general size and shape.

    摘要: The aspiration system with positive pressure includes a fluid flow network having a vacuum source at one end and a suction instrument at another end. Suction is regulated by a control valve that can be actuated by a foot pedal. Suction communication between the vacuum source and the suction instrument can be prevented by placing the control valve in the closed position. A positive pressure input device is included in the fluid flow network and is also actuatable by the foot pedal to provide an impulse of positive pressure at the suction instrument. The positive pressure impulse is provided when the aspiration process is being hindered by blockage of the suction inlet end of the suction instrument. A puff of positive pressure is used to expel material from the suction inlet end of the suction instrument. The use of a positive pressure impulse at the suction end of a suction instrument is also applicable to release material transported by the suction instrument from one location to another.
