A stapling assembly comprising a tissue thickness compensator is disclosed. The tissue thickness compensator comprises a body portion comprising a porous material and a plurality of cavities defined in the body portion, wherein the cavities are aligned with forming surfaces of an anvil such that fasteners of a fastener cartridge are configured to at least one of the cavities when the fasteners are ejected from the fastener cartridge or capture the cavities and compress the cavities within the tissue thickness compensator when the fasteners are ejected from the fastener cartridge and formed by forming surfaces of the anvil. The stapling assembly further comprises at least one medicament positioned within each cavity prior to firing the stapler, wherein the medicament is different than the porous material.
Abstract:
A cutting blade for a surgical cutting and stapling instrument is disclosed therein. The cutting blade can include one or more features to direct or displace substances released from within the tissue thickness compensators used with the surgical instrument. The cutting blade may include a textured surface that can cause substances from the tissue thickness compensator to spread across the blade surface.

Various tissue thickness compensators are disclosed for deployment between a stapler head of a surgical circular stapler and an anvil attached thereto to accommodate variances in tissue thickness during stapling. Some tissue thickness compensator arrangements include means and configurations for deploying healing agents for enhancing the healing process.

In various embodiments, a compensator can be attachable to an anvil of a fastening instrument. The compensator can comprise a support layer configured to be attached to the anvil and a scaffold attached to the support layer. In at least one embodiment, the scaffold can comprise a plurality of scaffold layers comprised of a biocompatible material and a plurality of cavities, wherein the layers and the cavities can define a matrix favorable to tissue and cellular ingrowth.

A staple cartridge assembly for use with a surgical stapler. The assembly has a cartridge body having a support portion with a plurality of staple cavities with openings. There is also a plurality of staples, wherein at least a portion of each the staple is removably stored within a the staple cavity. Each the staple is movable between an unfired position and a fired position, and is deformable between an unfired configuration and a fired configuration. The assembly also includes a compressible tissue thickness compensator configured to be captured within the staples. The compressible tissue thickness compensator at least partially covers the staple cavity openings. The compressed tissue thickness compensator is configured to assume different compressed heights within different the staples. Surgical end effectors are disclosed having angled tissue-contacting surfaces. The end effectors may have a first jaw member that is movable relative to a second jaw member between an open position and a closed position. The first jaw member may have a first positively-angled tissue-contacting surface. The second jaw member may have a second positively-angled tissue-contacting surface. At least one of the jaw members may have at least one active electrode configured to deliver RF energy to tissue located between the first jaw member and the second jaw member when in the closed position.

A two-part tissue thickness compensator assembly can include a first tissue thickness compensator configured to be positioned relative to an anvil of a surgical stapler, a second tissue thickness compensator configured to be positioned relative to a staple cartridge of the surgical stapler, and a hinge connecting the first tissue thickness compensator to the second tissue thickness compensator. The first and/or second tissue thickness compensators may include additional engagement features, such as a raised ridge that engages a slot in the anvil and/or the staple cartridge. In certain embodiments, the first and/or second tissue thickness compensators may include an encasement that contains a suitable biologic agent. An end effector assembly may be provided for attachment to a surgical instrument that includes, for example, a staple cartridge, an anvil, a first tissue thickness compensator positioned on the anvil, and a second tissue thickness compensator positioned on the staple cartridge.

A fastener cartridge can comprise a support portion, a tissue thickness compensator positioned relative to the support portion, and a plurality of fasteners positioned within the support portion and/or the tissue thickness compensator which can be utilized to fasten tissue. In use, the fastener cartridge can be positioned in a first jaw of a surgical fastening device, wherein a second jaw, or anvil, can be positioned opposite the first jaw. To deploy the fasteners, a staple-deploying member is advanced through the fastener cartridge to move the fasteners toward the anvil. As the fasteners are deployed, the fasteners can capture at least a portion of the tissue thickness compensator therein along with at least a portion of the tissue being fastened.
