摘要: The patent is relative to a new instrument for single access laparoscopy that tries to overcome the limits of the present instrumentation used in this technique, that usually allows to use only one forceps, which does not allow to keep the tissues tenses to work with the surgical instrument. To overcome this limitation, the proposed instrument presents two arms, arranged in parallel presenting a diameter equal to half of the main body in the opening direction of the same, while in the direction perpendicular to this these arms can have an higher diameter, provided its shape be entirely circumscribed by the external dimension of the initial body, and each bearing at the tip a forceps, which may be mounted directly on the arms or on two auxiliary arms. The internal mechanism allows opening the two arms, not necessarily in the plane of rotation of the control rings, while the forceps bend toward the inside, keeping their configuration, being all this controlled by a single command. Two additional commands allow then to open and close the forceps individually, and each command can be locked in any position. The forceps can be opened in any direction relative to the plane of the arms, which can also be adjustable in a particular version, as well as it is possible that one or both of the forceps are replaced by a tool of a different type. The instrument can also be made with curvature of the body opposite to that of the arms, the latter being also controlled with a further command. Finally, there are versions of different pliers with robotic motion control and implementation.

摘要: Devices and methods for treatment of a patient's vasculature with some embodiments configured for delivery with a microcatheter for treatment of the cerebral vasculature of a patient. Embodiments may include a self-expanding resilient permeable shell having a radially constrained elongated state configured for delivery within a catheter lumen, an expanded state with a globular and longitudinally shortened configuration relative to the radially constrained state, and a plurality of elongate filaments which are woven together, which define a cavity of the permeable shell Some embodiments may include at least about 40% composite filaments having a high strength material and a highly radiopaque material. In some embodiments, the total cross sectional area of the highly radiopaque material is between about 11% and about 30% of the total cross sectional area of the plurality of elongate elements.

摘要: Described herein are endoscopic staplers and methods used to apply one or more fasteners to body tissue. In one embodiment, a fastener-applying device, which is preferably a stapler, is passed transorally into the stomach to plicate stomach tissue by engaging tissue from inside the stomach and drawing it inwardly. In the disclosed embodiments, the tissue is drawn into a tissue chamber, causing sections of serosal tissue to be positioned facing one another. The disclosed staplers allow opposed sections of tissue to be moved into contact with another, and preferably deliver staples for maintaining contact between tissue sections at least until serosal bonds form. Each of these steps may be performed wholly from the inside of the stomach and thus can eliminate the need for any surgical or laparoscopic intervention, After one or more plications are formed, medical devices may optionally be coupled to the plication(s) for retention within the stomach.

摘要: A forceps is provided. The forceps includes a housing having a shaft that extends therefrom. The bipolar forceps also includes a hydraulic mechanism that includes a fluid line and a plunger operatively coupled to the fluid line. The plunger is translatable within at least a portion of the shaft from a proximal position to a distal position. An end effector assembly is operatively connected to a distal end of the shaft and includes a pair of first and second jaw members biased in an open configuration. Each of the first and second jaw members configured to receive at least a portion of the plunger when a fluid is caused to flow within the fluid line such that the first and second jaw members move from an open position for positioning to a closed position for grasping tissue.

摘要: A support apparatus for supporting a trocar while the trocar extends through a body wall of a patient includes an inflatable collar extending around the trocar which can be inflated to a predetermined size by a source of fluid where the source of fluid is located on the trocar support apparatus itself so as to be carried thereby and is defined by a pump mechanism to provide a fixed volume allowing inflation only to a fixed size. An abutment member is shaped to be received on an outer surface of the trocar sleeve and adjustable longitudinally of the trocar sleeve so as to be located at a selected position. A tube connecting the pump on the abutment to the inflatable collar is wrapped helically around the sleeve of the trocar. The collar includes a sleeve portion which can be unrolled on to the trocar sleeve.
