摘要: A surgical stapling instrument (1) comprises a body portion (2, 3), a handle (4) and a staple fastening assembly (8). The staple fastening assembly (8) includes a curved cartridge (10), which comprises at least one curved open row of staples, and a curved anvil (22), which is adapted to cooperate with the cartridge (10) for forming the ends of the staples exiting from the cartridge (10). The staple fastening assembly (8) is adapted to allow unobstructed access towards the concave inner faces of the cartridge (10) and the anvil (22). The cartridge (10) can be moved towards the anvil (22) from a spaced position for positioning tissue therebetween to a closed position for clamping the tissue. Preferably, a knife is contained within the cartridge (10) and is positioned such that there is at least one row of staples on at least one side of the knife.

摘要: An articulating surgical stapler having a proximal end and a distal end includes a handle at the proximal end and an end effector at the distal end for selective articulation to improve access to tissue requiring treatment and ease of use of the present linear surgical stapler. A support shaft connects the handle to the end effector. An articulation mechanism is positioned between the support shaft and the end effector permitting selective movement of the end effector relative to the support shaft.

摘要: A surgical instrument for surgically joining tissue includes a handle assembly, an elongate member extending distally from the handle assembly, an end effector disposed adjacent a distal portion of the elongate member, and an actuation assembly operatively associated with the handle assembly. The actuation mechanism includes a thrust bar disposed at least partially within the elongate member and a distal slide member. The thrust bar has a proximal portion, a distal portion, a first lateral side and a second lateral side and is configured to move between proximal and distal positions with respect to the handle assembly. The distal slide member is positioned adjacent the first lateral side of the thrust bar and is adjacent the distal portion of the thrust bar. The proximal portion of the distal slide member is adapted to slide relative to the thrust bar when thrust bar moves in a curvilinear direction.

摘要: A surgical stapler having a selectively bendable end effector is provided. A staple holding jaw of the end effector is coupled to another jaw for grasping tissue to be stapled. The stapler can adapted to drive staples into tissue having a pattern conforming with the selected shape of the end effector. In particular, the end effector can be adapted to drive staples in a linear pattern when the end effector orients staples in a substantially linear configuration. The end effector can also be adapted to drive staples in a non linear pattern when the end effector orients staples in a non linear configuration. Methods of stapling tissue with a stapler having a bendable end effector are also discussed.

摘要: A full thickness resection device comprises a control handle including an actuator wherein, when the device is in an operative position within a body lumen of a patient, the control handle remains outside the patient's body and a working head assembly coupled to a control handle by a flexible sheath, wherein, when the device is in the operative position; the working head assembly is located within a body lumen of the patient adjacent to a portion of tissue to be treated, the working head assembly including a tissue stapling mechanism including first and second tissue stapling members moveable relative to one another in combination with a first cable extending from the actuator through the flexible sheath to the first tissue stapling member so that, when the actuator is operated to draw the first cable proximally from the sheath, the first tissue stapling member is moved in a first direction relative to the second tissue stapling member.

摘要: Described is a full-thickness resection system which includes a control unit coupled to a proximal end of a flexible endoscope. The control unit remains outside of a body when the stapling head is in an operative position within a body lumen. The control unit includes (i) an anvil actuator coupled to an anvil in the stapling head, actuation of the anvil actuator moves the anvil axially relative to a stapling mechanism in the stapling head to compress a folded full-thickness portion of lumenal tissue between the anvil and the stapling mechanism. In addition, the control unit includes (ii) a stapler actuator coupled to the stapling mechanism in the stapling head, actuation of the stapler actuator causing the stapling mechanism to drive staples through the folded lumenal tissue against the anvil. Also, the control unit includes (iii) a tissue cutter actuator coupled to a tissue cutter in the stapling head, actuation of the tissue cutter actuator causing the tissue cutter to resect portions of the folded lumenal tissue.

摘要: A surgical fastening and cutting assembly having a housing defines a passage and a pusher, with a cam surface, slides in the passage. An arm couples to the housing and an anvil base is located on the arm. A body slides on the arm between the housing and the base. The body defines a bore and a staple passage having a staple therein. A cam follower extends from the cartridge body to the housing. In an open position, tissue can be placed between the anvil base and the body. In an intermediate position, the cam surface of the pusher can move the cam follower into a locked position and, thereby, the tissue can be clamped. In a fastened and cut position, the pusher can move such that the staple can be been driven and formed in the tissue and a knife, coupled to the pusher, can cut the tissue.

摘要: A surgical stapling instrument having a staple fastening assembly including a curved cartridge device. The device has at least one curved open row of staples having a first and second ends and having a concave side and a convex side. Opposite to the cartridge device there is a curved anvil for forming the ends of the staples. The instrument also includes a moving device adapted to move the anvil with respect to the cartridge device in a parallel relationship. The instrument also includes a staple driving device adapted to drive the staples out of the cartridge device towards the anvil, and a knife. The knife has opposing first and second sides and is contained in the cartridge device. There is also a knife actuating device adapted to move the knife towards the anvil. The instrument includes staple fastening assembly having a retaining pin which moves between the cartridge device to align them. The retaining pin is located on the second side of the knife in an intermediate region between the first end and second ends of the curved row of staples.

摘要: A method for performing trans-anal resection is achieved by applying a first stitch about the rectum, wherein deep bites of the first stitch capture and fold a rectum wall, creating a longitudinal staple line in the rectal wall at a position adjacent the first stitch, positioning a curved surgical stapler instrument in the rectum for capturing the rectal wall held by the first stitch, firing the surgical stapler instrument creating a circumferential first staple line approximately along the arc defined by the first stitch, and repeating the procedure about the circumference of the rectum until a predetermined portion of the rectum is fully resected.
