Abstract: The present disclosure generally relates to devices for ophthalmic procedures, and more particularly, to apparatus for performing subretinal injections and methods of use thereof. In some embodiments, a surgical instrument for fluid injection may include a handpiece configured for grasping by a user, the handpiece comprising: a first lumen disposed therein, the first lumen configured to receive a fluid cartridge comprising one or more injection fluids and a fluidic drive system disposed within the first lumen, the fluidic drive system for driving a flow of the one or more injection fluids from the fluid cartridge into a cannula. The cannula may be coupled to the handpiece and be configured to be introduced into an eye. The cannula may include a second lumen extending therethrough for receiving the one or more injection fluids flowed from the fluid cartridge and a needle movably disposed within the second lumen, the needle configured to extend from and retract into the second lumen at a distal end of the cannula.

Abstract: A surgical instrument includes a flexible outer grasping member and a flexible inner grasping member for peeling a membrane from the retina of a patient's eye. The surgical instrument includes a handle and at least one actuator. An outer tube has a proximal end mounted to the handle. The outer and inner grasping members extend outwardly from a distal end of the outer tube, or are configured to extend outwardly from the distal end of the outer tube. The at least one actuator may be used to move at least one of the outer grasping member and the inner grasping member relative to the other such that the inner grasping member and the outer grasping member are brought together to grasp the retinal membrane.

Abstract: A surgical tool for eye surgery that utilizes a stiffening sleeve and employs a needle of extendible reach. The reach off the needle into the eye is dynamically adjustable by forcible interaction between the stiffening sleeve and a cannula at an outer location of a patient's eye. An end of the tool may retract to facilitate extension of the needle for added reach. Additionally, the sleeve may be further independently retracted for even greater reach of the needle implement toward a surgical site within the eye of the patient.

In some embodiments, the apparatus includes an injection needle having a proximal end and a distal end, the distal end configured to be insertable into the subretinal space at a position on a surface of a retina. The apparatus may further include an inserter device removably coupled to the injection needle and a tubing having a distal end coupled to the proximal end of the injection needle and a proximal end coupled to a fluid source. The tubing may have a first lumen and a second lumen, wherein the tubing is disposed through the inserter device. The apparatus may further include a stabilizer configured to immobilize the injection needle at the position on the surface of the retina and a fluid source. The fluid source may include both a first fluid reservoir containing a non-treatment solution and a second fluid reservoir containing a treatment solution. The fluid source may be configured to provide the non-treatment solution from the first fluid reservoir to the subretinal space via the first lumen. The fluid source may be further configured to provide the treatment solution from the second fluid reservoir into the subretinal space via the second lumen.

Abstract: An ophthalmic surgical instrument for delaminating a retinal membrane includes a handpiece and an actuator mounted on the handpiece. An outer tube has a proximal end mounted to the handpiece. A grasping structure is extendable outwardly relative to a distal end of the outer tube responsive to movement of the actuator. The grasping structure includes a cavity having an opening facing the retinal membrane in use. A blade extends along a portion of the opening and a cushion defining a plurality of openings is positioned within the cavity. Vacuum pressure is applied to a channel in the cushion and coupled to the openings in order to grasp the retinal membrane, which may then be peeled from the retina.

Abstract: In certain embodiments, an apparatus is provided for performing a subretinal injection into a subretinal space between a retina and a retinal pigment epithelium of an eye. The apparatus includes an injection needle having a proximal end and a distal end, the distal end configured to be insertable into the subretinal space at a position on a surface of the retina. The apparatus includes a multi-lumen tubing having a distal end coupled to the proximal end of the injection needle and a proximal end coupled to a fluid control unit. The apparatus includes a stabilizer configured to immobilize the injection needle at the position on the surface of the retina. The fluid control unit has a plurality of fluid reservoirs containing a non-treatment solution, a treatment solution, and a working fluid which are injectable into the eye via separate lumens of the multi-lumen tubing.
