摘要: An ophthalmic laser system includes a laser beam delivery system and an eye tracker responsive to movement of the eye operable with a laser beam delivery system for ablating corneal material of the eye through placement of laser beam shot on a selected area of the cornea of the eye. The shots are fired in a sequence and pattern such that no laser shots are fired at consecutive locations and no consecutive shots overlap. The pattern is moved in response to the movement of the eye.

摘要: A method and apparatus for correcting vision, including a corneal-pocket keratome device to create a corneal pocket and a lens to be inserted and retained in the corneal pocket to effect correction. The corneal-pocket keratome includes a drive unit having cutting head elements which contact the subject eye during corneal pocket formation. The cutting head elements may be removeable and may be disposable. The cutting head elements include a corneal restraint device, which may be a positioning ring to position an eyeball with the cornea protruding through the ring; a keratome blade assembly with a corneal-pocket blade; and may also include an applanation shoe surface to restrain the cornea, in addition or instead of the positioning ring. The applanation shoe may be pivotable away from the surgical area. The corneal-pocket blade may include a guide which travels with the blade. The blade assembly oscillates laterally while extending forward into the cornea to form the pocket, and the amplitude of the lateral oscillation is preferably increased as the blade goes beyond an opening incision into the cornea. Lenses for this invention preferably include a feature to impede accidental lens movement after the lens is disposed within the corneal pocket, which may be a swelling after insertion or a circumferential irregularity. Lenses may be of Fresnel or non-Fresnel type, and may employ annular changes in the index of refraction of the lens material, as well as changes in refractive shape which may be annular or not, to effect variations in focal length for relieving presbyopia, astigmatism, and combinations of those as well as myopia and hyperopia. Drive control and vacuum for the positioning ring are provided under user command by a control unit having user inputs.

摘要: The invention provides improved structures, systems, and methods for supporting the optical elements of a microscope relative to the optical train of a laser surgery system. As the field of view of the microscope is substantially fully determined by the position of the objective lens, the laser delivery optics and the microscope can be aligned with a target location of the patient's eye by accurately aligning just the objective lens with the delivery optics. By structurally separating the objective lens from the other optical components of the microscope, and by maintaining accurate alignment between the objective lens and the laser delivery optics with a simple, tight-tolerance support structure, the remaining optical components of the microscope can be allowed to “float” relative to the objective lens with a looser-tolerance without degrading the operator's ability to align, observe, and optically direct an ophthalmic laser procedure.

摘要: A non Q-switched erbium laser operating on the I11/2→I13/2 transition produces laser output pulses characterized by a single spike containing approximately 90% of the energy, rather than a series of multiple spikes. The laser includes an optical gain medium with erbium being the lasing species, such as an erbium doped YAG rod. The gain medium is pumped with a flashlamp, with the voltage applied to the flashlamp and the duration of the applied voltage being carefully controlled to produce the single spike laser output. Depending upon the operating conditions, the full width half maximum (FWHM) of the spikes can be varied between 0.1 and 10 microseconds. The output of the erbium has fewer than 10 transverse modes. Output from the erbium laser may be directed onto corneal tissue in a preferred corneal sculpting procedure, in which the ablation rate is limited to no more than about 3 microns per laser pulse.

摘要: A method, apparatus and system for a transverse tracker is described that can greatly improve the accuracy, speed, range, reliability, versatility, safety, and efficacy of interventions such as laser microsurgery, particularly ophthalmic surgery, and industrial micromachining. The instrument and system disclosed are applicable to those situations wherein the positioning accuracy of laser lesions is critical, and/or whenever precise operations on a target or series of targets subject to random movement during a procedure are to be effected. The present invention provides means for stabilizing the motion of targets in a plane perpendicular to the axial direction, thus allowing an imaging system, diagnostics illumination, and/or laser beam to maintain a lock on the target area, regardless of its movement. The invention also provides means for recording eye movements in real time and in which data can be stored and manipulated for the purpose of compensating for lateral target motion by either hardware or software means. Unique attributes provided in the tracking system include means for (1) sensing contrast in recognizable large scale boundaries such as the change between the cornea/sclera interface (limbus), thereby to determine the absolute location and orientation of these boundaries, all without having to resort to digital sampling techniques and (2) dual mode operation of an electronic control system compatible with all analog technologies, thereby substantially increasing the speed of operations over other, comparable digital method. The tracking system of the invention is at least comprised of illumination sources, imaging optics, a sensor such as a position-sensitive detector, a movable optical element such as a mirror, a two-dimensional logic board, and a dedicated microprocessor, including appropriate signal processing firmware and software. Additional optics can be incorporated to provide interface with other assemblies such as depth tracking, target viewing and/or laser surgery subsystems. The system of the invention affords considerable flexibility in selecting particular components such as the illuminators, detectors and servo devices, which, in turn, allows broad application of this system to tracking tasks in diverse medical surgical, diagnostic, and industrial settings.

摘要: A method of correcting the refractive error in a cornea of an eye, including forming a flap in the surface of the cornea, and moving the flap to expose first and second internal surfaces of the cornea. An implant is positioned adjacent the first internal surface, at least a portion of the implant having refractive properties and held adjacent the first internal surface using a tool. At least a portion of the implant is ablated with a light emitting device, when the implant is positioned on the first exposed surface of the cornea and being held by the instrument. The flap is then repositioned over the implant.

摘要: A method for enhancing the accuracy of PRK wherein a UV power meter is placed in the optical path of the laser beam. In one preferred embodiment, the power meter is placed distal to the last optical element so that any optical degradation that affects laser performance is taken into account. The meter consists of a UV-B cube and a pulnix camera with a software package. The meter is used to monitor the fluence of each laser pulse. The power meter is used to size each pulse and to quantify the energy in each pulse. Sensing means is employed to measure intraoperative pulse-to-pulse energy during photorefractive keratectomy (PRK), using said data in conjunction with the location of the pulse within the ablation zone to determine the cumulative energy thus being achieved, and adjusting said laser to treat more or less at each point based upon the difference between the ideal cumulative energy map and the observed cumulative energy map derived from intraoperative power determination.
