摘要： The method of the present invention comprises placing a test result printout on a work sheet having a grid thereon comprising a vertical and horizontal matrix of columns and rows. The rows are sized and spaced to register with the individual indicia in the column of indicia on the test printout. After the indicia on the test printout are aligned and registered with the horizontal rows on the work sheet, the indicia are transcribed from the printout to the work sheet. A data slip is then laid over the work sheet. The data slip has a horizontal row of boxes sized and spaced to register with a group of columns on the work sheet. The data slip is positioned so that the row of boxes is adjacent one of the horizontal rows on the work sheet and is registered with the columns within said group. The indicia in the columns are then transcribed from the columns to the boxes. Finally, the data slips are adhered to a patient chart in a shingled fashion so that the boxes of each data slip are exposed on the lower shingled edge of the data slip.

摘要： A system of movable headings for bookkeeping includes a cardboard heading strip slidable into transparent plastic envelopes, and the envelopes which are glued to pages of record books. Headings need be written on the heading strip only once. The heading strip then can be moved to another book, or to the next sheet in the same book, for reuse. In a sheaf of pages, the envelope can be moved from a top page to the next page and the page area occupied by the envelope on the previous page removed. A paper cover envelope, open at one end, has carbon paper inside and an adhesive backing; the heading strip is inserted and the headings written on the outside of the envelope. The headings are transferred to the heading strip by the carbon. The paper envelope serves as a record and title of the headings.

摘要： Conditionally self-copying stationery is provided wherein a carbon copy of handwritten material can be made dependently upon the attitude of insertion of a copy sheet. The copy sheet has a carbon back coating on one face thereof and each of the writing sheets in a pad has a carbon front coating on the upper surface thereof. Dependently upon whether the copy sheet is inserted such that its carbon back coating makes contact with a carbon front coating or not, so a carbon copy is selectably made or not made. The copy sheet is the conventional writing guide found in writing pads, and is printed on both sides with writing guidelines.

摘要： An information gathering system particularly useful for obtaining medical information from a patient, including patients which communicate in a language different from that of the doctor. The system has one or more question sheets bearing a set of questions. These questions may be medical questions written in the language of the patient. A separate answer sheet on which answers to questions on the question sheets can be indicated is provided. The answer sheet has information concerning each possible answer adjacent the location on the sheet for the answer. It is possible for this information to be in a language different from that used on the question sheet or sheets. A mechanism is preferably provided for positioning and holding the answer sheet in a predetermined position relative to the question sheets. In this way the questions are correctly positioned with respect to corresponding locations on the answer sheet for the answers to the questions so as to enable a person answering the questions to write or mark his answers at the proper locations.
