Abstract: A direct peroral cholangioscope system comprises a guide sheath sized to pass through an esophagus of a patient comprising a proximal end portion, a distal end portion, and a lumen extending between the proximal end portion and the distal end portion, an endoscope comprising an elongate body extending from a proximal end to a distal end, an imaging device coupled to the elongate body proximate the distal end, and a passage extending at least partially through the elongate body and exiting at the distal end, the elongate body sized to pass through the lumen, and a tissue retrieval device configured to pass through the passage and retrieve tissue from within the patient. The guide sheath can be used to steer the endoscope through the pyloric sphincter and the sphincter of Oddi to allow the endoscope to be articulated in the common bile duct using native steering capabilities.

Abstract: A controller for an auxiliary endoscope comprises a coupling piece for attaching to a main endoscope, a handpiece connected to the coupling piece, a working shaft extending from the handpiece and extending into the coupler and a first control feature located on the handpiece for operating a pull wire extending through the working shaft, the pull wire configured to steer the auxiliary endoscope, wherein the handpiece can be moved relative to the coupling piece to adjust a position of the auxiliary scope relative to the main scope.

Abstract: An endoscopy system can include or use an endoscopic overtube, sized and shaped for insertion into a patient esophagus towards a patient duodenum. The overtube can include and access lumen providing an inner diameter to receive and pass a cholangioscope therethrough. The distal portion of the overtube can include or use a side port sized and shaped to permit lateral exit of a distal portion of the cholangioscope. Also, the overtube can include a ramp arranged to re-direct the distal portion of the cholangioscope from a longitudinal direction in the access lumen toward the lateral exit of the side port.
