    摘要: A bone fixation system includes a plate and a set of fixation locking screws. The plate defines a set of locking screw holes each having an internal thread. Each respective locking screw has a head with an external structure that is adapted to self-tap into the internal thread of a given locking screw hole to secure the respective first-type fixation locking screw at an surgeon-directed angle relative to the plate. This angle is defined during forcible insertion and rotation of the respective locking screw into the given screw hole. The system may also include unidirectional locking screws. In a preferred embodiment the plate and first-type of screw are made of different metals, with the plate made of titanium alloy and the screw made of cobalt chrome alloy.

    摘要: A method of performing a treatment for gastroesophagheal reflux disease (GERD) includes providing a distal end of a shaft of a medical instrument with a proximally facing end effector having a laterally offset clevis defining a shaft-clevis plane and opposing mutually rotatable jaws proximally directed and laterally displaced relative to a longitudinal axis of the shaft. The jaws hold parts of a two-part fastener, respectively, and are offset and separated from the shaft. Significantly, the jaws operate in a plane at an angle to the shaft-clevis plane. The shaft is inserted with the proximally facing end effector through the esophagus in a first direction. The shaft is retracted in a second direction substantially opposite the first. The end effector contacts a stomach wall at a location near the esophagus and the stomach wall is manipulated with the end effector.

    摘要: A clip applier includes a flexible coil with a manual actuator coupled to one end and a jaw assembly coupled to the other end. A store of clips is adjacent to the jaw assembly and a clip pusher is adjacent to the store of clips. The actuator includes a lever for opening and closing the jaws, a knob for rotating the jaw assembly, and a crank for dispensing clips. The knob and the lever are coupled to a single control member which extends through the coil to a joiner where it is joined to a pair of pull wires coupled to the jaws. The crank is coupled to a second control member which is threaded along a portion. The threaded portion engages a threaded member coupled to the pusher such that rotation of the threaded control member by the crank causes the pusher to be moved distally.

    摘要: A surgical instrument includes an end effector having a clevis and first and second jaws mutually rotatable between open and closed positions. The jaws are proximally directed and laterally displaced relative to a longitudinal axis of a control shaft of the instrument. The jaws hold first and second parts of a fastener, respectively. The first part includes a base having upstanding tissue piercing posts, and the second part includes another base defining apertures for receiving the posts, as well as a portion movable relative to the second base. When the upstanding posts are inserted into the apertures, the movable portion can be moved into a second configuration to lock the parts of the fastener together. The instrument is adapted to move the second part into the second configuration. A method for using the apparatus and fastener are also provided.

    摘要: An electro-surgical device for performing tissue resection and cauterization includes an elongated body, a pair of arms extending from a distal end of the elongated body, and an electrode in communication with the pair of arms. The elongated body is adapted to be coupled to a source of energy at a proximal end. The electrode has a first region coated with an insulative coating and a second region for focusing energy emission. The coating can be a diamond-like carbon coating or other coating exhibiting resistance to cracking at high temperatures and high voltages. The coating can be deposited on a base section of a loop electrode while the end sections remain free of the coating. Alternatively, the coating can be unevenly deposited on the electrode. In other embodiments, the electrode can comprise a rollerball and the insulative coating can be deposited on selected or random portions of the rollerball. The insulative coating prevents energy loss to fluid and tissue during the operation, thus allowing resection and cauterization to be efficiently performed.
