An EMS medical record acquisition system is provided. The system is configured to provide medical record information to an EMS rescuer in a mobile EMS environment. The system includes a user device, a patient identification data source, and a medical record digest system. The digest system is coupled to the user device and to the patient identification source. The digest system receives identification data from the patient identification source, queries a medical record repository using the identification data, receives a medical record for the patient from the medical record repository, generates a reference code based on the identification data, stores the medical record with the reference code, transmits the reference code to the user device, receives a query including the reference code from the user device, generates a digest at least a portion of the medical record, and provides the digest to the user device.

A method for evaluating emergency medical service according to embodiments of the present invention includes receiving emergency medical service data from a database, filtering the emergency medical service data based on a selection criteria to form a filtered emergency medical service data set, determining a first score from the filtered emergency medical service data set, where the first score indicates objective clinical performance quality for the filtered emergency medical service data set, determining a second score from the filtered emergency medical service data set, where the second score indicates objective operational performance quality for the filtered emergency medical service data set, merging the first score and the second score to form a composite score; and visually displaying the composite score to a user.

A patient data management platform, e.g., a community paramedicine platform, which may also be referred to as a mobile integrative health platform or system, may include a web-based, cloud-based, and/or software-based system for tracking patients and patient visits by health care providers, paramedics or other emergency medical services (“EMS”) professionals in an out of hospital, mobile, and/or non-emergency context. Such a platform performs functions and data input, visit logging, and reporting tasks that are distinct from that of a traditional EMS electronic patient care reporting system.
