摘要:
An indirect calorimeter for measuring the metabolic activity of a subject includes a respiratory connector operative to be supported in contact with the subject so as to pass inhaled and exhaled gases therethrough as the subject breathes, and a flow tube forming a flow pathway for passing inhaled and exhaled gases therethrough, wherein one end of the flow tube is operatively connected to the respiratory connector and the other end of the flow tube is open, and a wall of the flow tube includes an opening. The indirect calorimeter also includes a flow meter adapted to generate a signal as a function of the instantaneous volume of inhaled and exhaled gases in the flow pathway that is in fluid communication with the flow pathway via the opening in the flow tube, and an oxygen sensor operative to generate a signal as a function of the instantaneous fraction of oxygen in the inhaled and exhaled gases in the flow pathway that is in fluid communication with the flow pathway via the opening in the flow tube. The indirect calorimeter further includes a processor for receiving the signals from the flow sensor and the oxygen sensor and using the signals to determine the oxygen consumption of the subject over a period of time.

The invention relates to an inexpensive method in which the partial pressure of a gas component in the air exhaled by a patient is determined per breath.

In a meter for assessing ventilatory capacity, a piston is free of any sliding contact with the body of the meter except over a region which extends axially not less than about 25% of the piston's diameter. In this way, tilting of the piston is avoided. Sliding contact is further reduced by the piston or wall having contact ribs extending axially.

An indirect calorimeter operative to measure the respiratory oxygen consumption per unit time of a subject includes a respiratory connector operative to be supported in contact with a subject, so as to pass respiratory gases as the subject breathes into the respiratory connector, and a bi-directional flow meter having an ultrasonic flow transducer that bi-directionally transmits and receives ultrasonic signals to the transducer to generate a signal as a function of the volume of gases passing through the flow meter. The indirect calorimeter also includes a gas concentration sensor operative to generate a signal as a function of an instantaneous carbon dioxide content of gases passing by the gas concentration sensor. The indirect calorimeter further includes a computing device operative to receive signals from the flow meter and the gas concentration sensor, and conduits interconnecting the respiratory connector, the flow meter and the gas concentration sensor, so that the subject's inhalations and exhalations pass through the flow meter and the subject's exhalations pass over the gas concentration sensor and the computer is operative to receive the signals from the gas concentration sensor and the flow meter to calculate the subject's oxygen consumption.

A method for prescribing an exercise regimen for a particular subject to either maximize cardiovascular performance or to lose weight involves the use of a microprocessor-based cardiopulmonary exercise system to measure oxygen uptake and carbon dioxide production on a breath-by-breath basis. These measured quantities are used to calculate energy expenditure and a subject's respiratory exchange ratio from which a fat metabolization curve can be plotted. By examining the length of a plateau in the curve where the fat substrate utilization is maximized, and noting the average heart rate in this zone, a target heart rate for optimal weight loss is arrived at. If the goal of the exercise is cardiovascular improvement, the anaerobic threshold can readily be determined as the midpoint between maximal fat utilization and the maximum acceleration towards the point where the respiratory exchange ratio becomes equal to 1 or the fat metabolism goes to 0. By noting the average heart rate at the anaerobic threshold, a target heart rate for optimizing cardiovascular fitness may be arrived at.

A patient monitoring apparatus for quantatively measuring a physiological characteristic of a patient. A first patient interface communicates with an airway of a patient such that substantially all gas inhaled and exhaled by the patient passes through the first patient interface. One or more vent elements associated with the first patient interface communicate the first patient interface with an ambient atmosphere so that a pressure differential is created between the pressure in the first patient interface and the pressure of the ambient atmosphere. A sensor communicates with the first patient interface and measures a fluid characteristic resulting from this pressure differential and outputs a first signal indicative of that characteristic. The output from the sensor is used alone, or in combination with the output from other sensors that detect other physiological characteristics, to provide a variety of information about the patient.

A noninvasive apparatus and method for measuring a subject's end tidal carbon monoxide concentration is disclosed, featuring the ability to (i) determine carbon monoxide concentration on a breath-by-breath basis, (ii) determine mean carbon monoxide concentration by averaging local carbon monoxide values, (iii) avoid the premature determination that an end tidal phase is over, and (iv) determine when breath variability or system variability will likely preclude accurate testing. The disclosed device and method allow for more robust determination of end tidal carbon monoxide concentration in subjects with turbulent or irregular breath patterns.

A respiratory gas analyzer for measuring the cardiac output of a subject includes a flow meter and an oxygen sensor interconnected with one another between a mouthpiece and a source of respiratory gases, which may be a controlled source or the atmosphere. An oximeter provides measurements of the oxygen saturation of the subject. A computer connected to receive the signals from the flow meter, oxygen sensor, and oximeter can then calculate the subject's cardiac output.

A combination thermal and vibration sensor for use in sleep monitoring equipment comprises a thin film of a plastic exhibiting both pyroelectric and piezoelectric properties. By providing a layer of foam material totally covering one major surface of the plastic film layer, it is found that the thermal mass of the sensor is changed to the point where the pyroelectric signal can more readily be isolated from the piezoelectric signal using conventional signal processing techniques.

Disclosed is a neck seal for use in a volume displacement plethysmograph for investigating pulmonary function of infants. The neck seal includes particulate material entrapped in a space between two layers of flexible sheets. The two flexible sheets have aligned openings to allow an infant's neck to be located therein. The sheets are sealed together around the openings and along a closed figure extending around the openings, thereby forcing the space closed. The seal becomes rigid when a vacuum is applied to the space.
