Therapeutics and methods of treatment to repair, preserve and grow cartilage are presented. In addition, systems and methods for delivering a therapeutic to a hard to reach anatomical area, such as, for example, the BCI, are presented. A cannulated delivery device provided with a cutting tip, cutting flutes and threads on its distal end is disclosed, as well as therapies for joint and cartilage repair, preservation and generation using it. Alternatively, for disc repair, a “PIARES” device for Percutaneous Intradiscal Annular Repair introduces therapeutics intradiscally. The device may have two-needles; a first cannula/needle with a finger grip, and a longer inner needle to penetrate through the outer needle into the disc, and introduce therapeutics via a syringe. When provided with a septum at the inner needle's proximal end, the PIARES device is a completely closed system; using it minimizes trauma.

A wearable iontophoresis system includes a control housing having a connector and an iontophoretic drug delivery patch for placement against the skin of a patient. The patch has a device for retaining the control housing connector which permits the connector to be removably attached to the patch. The control housing also has at least a power supply device and a current delivery device. The control housing has top and bottom surfaces wherein the top surface has raised, recessed and sloping regions. The thickness of the connector is tapered from a second end to a first end. Furthermore, the second end of the connector is attached to the control housing and oriented so that the first end is unattached to the control housing and proximate to the control housing bottom surface. The iontophoretic drug delivery patch includes top and bottom surfaces wherein mounted to the top surface is the device for retaining the control housing connector. The control housing connector retaining device has first and second open ends and a cavity for placement therein of the connector. The taper of the connector permits it to be easily inserted through one of the first and second open ends and into the control housing connector retaining device cavity.
