摘要： A mailing form, including an upper sheet of material adhesively fastened to a lower sheet, is configured for the printing of information on a single side during passage of the form through a non-impact printer. At a first end of the form, the upper and lower sheets are fastened together permanently, forming a pocket to be used as a return envelope. The portion of the upper sheet which extends away from this pocket is divided into an inner flap, on which information is printed, and an outer flap, which is temporarily fastened to the lower sheet in order to protect an adhesive pattern during the printing process. After the printing process, the outer flap is removed and discarded, the inner flap is folded along lines provided for folding, and the lower sheet is folded on itself to enclose the inner flap. As the adhesive pattern holds the lower sheet shut in this position, address and return address information printed on the inner flap is visible through apertures in the lower sheet, which forms an envelope. After receiving this envelope, the recipient separates the pocket forming a return envelope from the rest of the materials. A part of the inner flap may be returned in this return envelope, together with a check or other remittance as desired. The address and return address has been printed on this envelope during the printing step. Before the return envelope is mailed it is sealed using a flap with remoistenable adhesive.

摘要： A carbonless form is provided that may be folded selectively into a duplicating state or a non-duplicating state. In accordance with one embodiment of the present invention, a carbonless form is provided comprising a form sheet, a transverse fold line, an assigned data entry portion, and an exposed data entry portion. The form sheet defines a front face, a rear face, a top edge, a bottom edge, and a pair of side edges. The transverse fold line extends across the form sheet to define a first panel and a second panel on opposite sides of the transverse fold line. The assigned data entry portion is defined on the rear face of the second panel. The exposed data entry portion is defined on the front face of the first panel. A CB coating is applied to the rear face of the form sheet in the first panel and a CF coating is applied to the rear face of the form sheet in the second panel. The transverse fold line, the CB coating, and the CF coating are arranged such that the form sheet may be folded selectively about the transverse fold line into either a duplicating or a non-duplicating state. The duplicating state is characterized by juxtaposition of the CB and the CF coating. The non-duplicating state defines a common printing face including the assigned data entry portion of the second panel rear face and the exposed data entry portion of the first panel front face.

摘要： A reusable envelope has a front address panel and a rear panel joined along three edges to form a pocket with an open top. A first extended sealing flap joined to the top edge of the address panel is provided with a tear strip formed by a line of perforations extending across the top edge of the envelope and through the flap. A second sealing flap is provided which is foldably connected to the top edge of the rear panel and is initially either folded and retained inside the envelope pocket or against the exterior of the rear panel for the first mailing, and is folded over and onto the first panel for the second use. The second flap has an adhesive area securing means for sealing the envelope when the flap is positioned on the address panel for a further mailing. The address panel is provided with an area is for postage indicia and, is optionally provided with a first return address area adjacent the top edge of the address panel. The address panel optionally includes a second return address area located adjacent the bottom edge of the address panel and another area for applying postage. The second sealing flap is sized to cover at least the first return address area and first postage area and, optionally, the first address area, when the second sealing flap is folded over and sealed to the address panel, and can also be pre-printed by the originator with return address and bulk mailing indicia. A single-use easy-open envelope employing the tear strip is also described.
