摘要: A pivotal bone anchor assembly includes a receiver having a channel configured to receive an elongate rod and communicating with a bottom opening through a central bore, with the central bore having a lower portion configured to receive the capture portion of a bone anchor with the bone anchor being pivotal with respect to the receiver in a non-locked configuration. The assembly further includes a compression insert that is also receivable within the central bore with an upper surface configured for engagement with the elongate rod, a central opening, and upward-facing surfaces on tool engagement structures positioned radially outward from the central opening. The tool engagement structures are configured for direct engagement by tooling to receive a continuously-applied downwardly directed force that temporarily locks the position of the bone anchor with respect to the receiver prior to the elongate rod being secured within the channel in a locked arrangement.

摘要: A method of assembling a pivotal bone anchor assembly includes positioning a retaining structure into a central bore of a receiver having a planar seating surface above a lower opening, the retaining structure being operably slidably coupled with the planar seating surface and having a central opening, the receiver including upright arms defining an open channel configured to receive a rod and exterior surfaces comprising opposite cylindrical upper side surfaces defining a first width and opposite lower side surfaces below the upper side surfaces defining a second width. The method also includes positioning a partially spherical head of a shank into the central opening of the retaining structure, with the retaining structure being slidably movable along the planar seating surface of the receiver to provide for increased articulated movement between the shank and the receiver in at least one direction relative to a vertical centerline axis of the receiver prior to locking the assembly with a closure.

摘要: A pivotal bone anchor assembly includes a shank having a shank head and an anchor portion, and a receiver having a first channel for receiving a rod and an axial bore for receiving the shank head, with the axial bore including a downwardly-facing abutment surface and opposed rotation blocking and alignment structures beneath a helically wound thread. The assembly also includes a pressure insert having a second channel and opposite outwardly-projecting flanges with notches formed therein. The pressure insert is top loaded into a first position within the axial bore with the second channel in a mal-aligned position relative to the first channel, with subsequent rotation of the pressure insert moving the second channel into alignment with the first channel, the opposite outwardly-projecting flanges under the downwardly-facing abutment surface, and the opposed rotation blocking and alignment structures into the notches formed into the opposite outwardly-projecting flanges.

摘要: A spinal fixation system includes a plurality of bone anchors, each having an anchor portion and a shank head with an upper spherical surface having a first diameter extending downward to an inner edge of an annular shelf spaced below a hemisphere plane, and a lower spherical surface having a second diameter greater than the first diameter extending downward from an outer edge of the annular shelf toward a neck extending between the shank head and anchor portion. The system also includes both pivoting and non-pivoting receiver sub-assemblies, with each receiver sub-assembly including a receiver having a central bore with a seating surface adjacent a bottom opening and a channel for receiving a rod. Each receiver sub-assembly also includes one of a multiplanar retaining structure, a monoplanar retaining structure, or a monoaxial retaining structure positioned therein and slidably engageable with the seating surface after capturing the shank head upon its uploading through the bottom opening.

摘要: A method of assembling a receiver assembly includes aligning an insert with a receiver such that a second channel of the insert is in non-alignment with a first channel of the receiver, followed by loading the insert into a through-space of the receiver that extends between a bottom opening and an upper opening. The through-space includes an internal recess beneath a helically wound guide and advancement structure and an internal ridge projecting inwardly below the internal recess. The insert includes side structures protruding laterally outward and spaced below top surfaces. The method further includes rotating the insert within the through-space of the receiver to align the second channel with the first channel so as to together receive an elongate rod, while also rotating the side structures of the insert into the internal recess of the through-space with the side structures being adjacent to and above the internal ridge.

摘要: A polyaxial bone screw assembly includes a threaded shank body having an integral upper portion receivable in a receiver, the receiver having an upper channel for receiving a longitudinal connecting member and a lower cavity cooperating with a lower opening. The shank upper portion expands a retaining member in the receiver cavity to capture the shank upper portion in the receiver. The retaining member and attached shank are pivotable with respect to the receiver until locked in place with respect to the receiver. A pre-assembled receiver, retaining member and compression insert may be popped-on or snapped-on to the shank upper portion prior to or after implantation of the shank into a vertebra.

The assembly includes a shank having a capture portion with rounded shape including a spherical lower outer surface extending downward toward a neck, and an anchor portion opposite the capture portion. The capture portion is positionable into the central bore with the lower outer surface in pivotal engagement with the seating surface and the shank extending downward through the lower opening, and with the neck of the shank being positionable in the cut-out portion of the lower opening to provide for increased pivotal angulation of the shank with respect to the receiver.
