The tool engagement structures are configured for direct engagement by tooling to receive a continuously-applied downwardly directed force that temporarily locks the position of the bone anchor with respect to the receiver prior to the elongate rod being secured within the channel in a locked arrangement.

The pressure insert is top loaded into a first position within the axial bore with the second channel in a mal-aligned position relative to the first channel, with subsequent rotation of the pressure insert moving the second channel into alignment with the first channel, the opposite outwardly-projecting flanges under the downwardly-facing abutment surface, and the opposed rotation blocking and alignment structures into the notches formed into the opposite outwardly-projecting flanges.

The assembly includes a shank having a capture portion with rounded shape including a spherical lower outer surface extending downward toward a neck, and an anchor portion opposite the capture portion. The capture portion is positionable into the central bore with the lower outer surface in pivotal engagement with the seating surface and the shank extending downward through the lower opening, and with the neck of the shank being positionable in the cut-out portion of the lower opening to provide for increased pivotal angulation of the shank with respect to the receiver.
